Actions behave erratically when there are multiple fields with era on one form

Problem/Motivation

When having two seperate fields using entity_reference_actions, multiple problems arise.

• One field empty it will change the other fields entities.
• After adding a selection to both field, it still changes entities of only one.
• Notifications will show on the first instance.

Steps to reproduce

Configure two reference fields on one content type with ERA actions.

Proposed resolution

Make ERA aware of its context, i.e. the field and its selection it is displayed in.
