Token refresh gives exception; token time is not refreshed

Problem/Motivation

Refreshing a token (adding the same token twice via the JSON-RPC endpoint) fails with this error:

Cannot load the "pn_registration_token" entity with NULL ID.

The token time also fails to refresh because no fields are changed.
