Conditional initialisation options displayed regardless of state

Problem/Motivation

The option to set the text and description for the text box under 'Conditional initialization' is displayed regardless of the select option selected.

Steps to reproduce

Edit the settings of the map display formatter, under 'Conditional initialization' select the 'No' or 'Custom code' options and the text button text and description fields remain in place.
