Replace textarea field with two text fields

Currently, we use one textarea field to add themes and their labels in the next format:

customtemplate|Friendly Name
customtemplate2|Friendly Name 2

It should be replaced with two text fields (template name & theme label) with the possibility to add/remove (+ Add new button) new pairs of values.
