A single QR code file is used in 3 places

Create 1 Qr code entity, then go to Files and see the single svg "Used in" 3 places, then click on it, and you see this usage count:

(there is only 1 qr code entity and file, but somehow the rows are duplicated)

I have checked the issue & as you described it's showing duplicate rows after click on usages count. for solution i have applied Query Distinct in Files (File) view. added screenshot for reference, please review.
