Blank page on admin/structure/modal

Please provide a description and steps to reproduce the issue.

I saw that the description was in the "tags" field. So just moving to the description

P.S. I also tested changing the configs on "Global Settings" but continue working

2) Open the page "

" to see the "blank page"
3) Open the page: "/admin/reports/dblog"
4) Get the log of error and paste here, please

Let’s try to reproduce

I downloaded the latest version 5.0.4. I am still facing exactly the same issue as mentioned earlier. The error that I am getting is as follows;

I can able to replicate the same issue : blank page found here: /admin/structure/modal , when checked the checkbox : "Don't show again"
of Modal: "Thank you for installing Modal Page"

I guess for this cookie should maintain if user checked the "Don't show again" checkbox, instead of delete modal due to which modal stop displaying at modal listing page in the back-end & it gives impression to user that due to some config level changes page /admin/structure/modal goes blank
