In Opt-in mode, requireConsent/disableCookies flags are never set

Problem/Motivation

When EU Coookie Compliance is configured in Opt-In mode (ie without categories), the matomo flags 'requireConsent' and 'disableCookies' are not set, even if the user didn't give consent.

They are set only in 'Opt-In with categories' mode.

Proposed resolution

Adjust the generated javascript to test for consent mode before checking for categories.
