Cannot add new Activity Type

Problem/Motivation

When I try to add a new Activity Type in "Opigno Module", a blank page is displayed, with the following message:
"The website encountered an unexpected error. Please try again later."
See the attached screenshot.

Also, following error gets logged into the "Reports" -> "Log Messages" section:

Steps to reproduce

  • Go to "Opigno Module" -> "Activity Types" (URL: .../admin/structure/opigno_activity_type)
  • Click on "Add Activity type" button (URL: .../admin/structure/opigno_activity_type/add)
  • Fill in the required information and click "Save" button
