Mentions created every time a node is saved

Every time a node is saved a mention is created. Old mentions are not recognised correctly leading to this.

Create a mention in a node.
Edit and save the node without any changes.

2 changes exist in the database instead of 1

The problems seems to be in function `mentions_crud_update` where it uses `$entity->get('uid')->getValue()[0]['value']`

We were seeing the same duplicate Mention entity issues with our nodes. Whenever we edited or resaved a node a duplicate mention entity would be created. @lunatic, your patch fixed this issue for us. Nice work.
