how to add disable/enable option for each category and Accept/decline all

I think you only created categories and not services.
Categories are groupings of services:

Service 1
Service 2
Category 2:
Service 3
Service 4
To manage services, you can go here: admin/content/orejime_service
Categories are optional

Automatically closed - issue fixed for 2 weeks with no activity.
