Schedule maintenance disable date

Problem

Currently, we can only specify the enable date for maintenance mode, but there is no option to schedule a disable date.

Proposed resolution

Introduce a new date form element that allows scheduling the maintenance disable date.

User interface changes

Add a new date field to the existing maintenance mode form.
