Duplicate title fields and title value not showing on the modal

Problem/Motivation

- Title field is showing duplicated on the entity embed modal

Steps to reproduce

- Install entity_browser module and entity_embed_link module
- Configured, created an Image Media embed button in ckeditor
- Try to add an image
- Double click on the image to edit, you will see 2 issues below
+ The title fields are showing twice exactly the same.
+ Even if you fill out the two title fields, value of the title from entity_embed_link never shown up.
