Updated patch to make it not do anything in cases where the headings don't have a valid hierarchy (e.g. h3 before h2)

At the moment, the table of contents items have no numbering. Also, headings and subheadings are always on the same level in the table of contents.
Add two settings to the TOC block - Auto numbering and auto tabbing
