Switch markup strategy to use #theme_wrappers

Problem/Motivation and Resolution

Change the way that the markup, initially contained in html comments, is added around entities from [#prefix] and [#suffix], to use custom [#theme_attributes].
This still wraps entities in the correct markup, that might have their display altered in other ways, like in a preprocess hook.
