Some array items in form hooks don't exist and that causes warnings

Problem/Motivation

The 'actions' array item sometimes doesn't exist. And that causes PHP warning.

Example: "$element['subform']['field_module_lessons']['widget']['actions']['ief_add']['#value'];".

Proposed resolution

Always check the key for existence.
