Standardize on one name for this module.

Problem/Motivation

The name of this module is not consistent. The name for this module should be standardized.

Steps to reproduce

In the list of modules, this module appears as "Separate Password Form". I prefer this name, its better grammar. However,everyplace else I've seen this module, its title appears as "Password Separate Form".

Proposed resolution

I would like a choice to be made. I defer to the module creator to either name the module "Separate Password Form" or "Password Separate Form"
