Create a Cloudinary media source instead of a separate media type

Problem/Motivation

Existing solution is not ideal, it's hardcoded to use only one media type like Cloudinary Image. Flexible solution would be to create a dedicated media source so multiple media types can use it. In this case we don't extra fields and configs for media type during module installation. We let developers to set up own media type.

Proposed resolution/Remaining tasks

1. Create a new media source Cloudinary
2. Create a separate field to store raw transformation during media type creation
3. Adjust the code to work with a media source instead of hardcoded media type.
4. Create a migration path from existing solution to new media source.
