Accessibility problem with layout_paragraphs 2.x when inserting media via modal within modal

We're loving layout_paragraphs in the LocalGov Drupal distribution, thanks for all your work on it. We've been using the 1.x branch and are now moving to the 2.x branch and have been testing the user experience and accessibility with councils in the UK and would like to help fix a a few important issues.

I've tested this on the 1.x branch and it isn't an issue there.

Problem/Motivation

When you have a paragraph with a Media field, using Media Library, the keyboard focus does not move to the "Add or select media" modal.

This makes it impossible for keyboard-only users to add a media item to a paragraph within a layout_paragraph section.

Steps to reproduce

Assumptions
- A paragraph with a media field exists, and the form display is set to Media Library

1. Add a new piece of content
2. Add a new Section, choose a layout
3. Add an Image paragraph to the new section. Focus is moved to the new modal.
4. Click "Add media" - the focus is not moved to the the "Add or select media" modal.
