There are both a 4.0.x and a 4.0.x-dev branch. Recent issues seem to only be committed to the 4.0.x-dev branch, but d.o. expects the dev branch to not have the -dev part in the branch name. This results in merge requests being created from 4.0.x.
Merge all code from the 4.0.x-dev branch into 4.0.x and then delete the 4.0.x-dev branch.
