The change that went in with #3302942: Support node update and delete hooks to keep index up-to-date → doesn't account for nodes that are being published, but has not changed the title.
Add a new unpublished node, then publish it, but don't change the title. Note that the title is not added to the suggestions index.
Always re-index published nodes on update instead of only if the title has changed.
