Document that options are on the transition, not the state

Problem/Motivation

The project page and README for this module state:

Edit moderation states and check the Allow editors and Allow reviewers as
needed.

When I edit a state, however, I don't see these options. If I edit a transition, on the other hand, I do see these options:
Allow editors to make this transition
Allow reviewers to make this transition

Proposed resolution

Either update the documentation or make the module was as documented.
