I installed this on my site running Drupal 9.4.5 with PHP 8.1.8 via composer. I then enabled the module, made the needed change to settings.local.php (I also tried it by adding it directly above the call to settings.local.php) then proceeded to go to the URL /error_page_test/exception on the site and it replies with the stock 404 error page.
the line I added is per the documentation:
$settings['extension_discovery_scan_tests'] = TRUE;
Am I missing something?
