Minor issue of the menu item not being created. Unless I am reading the code wrong there should be a menu link that goes to /admin/config/development/subpathauto, however it is not being created.
Install on a new system that has not had it installed on it before. Or uninstall the module and reinstall it.

Can't replicate this.
The menu item should be under Configuration -> Search and Metadata -> [Sub-path Settings] in the admin toolbar.
