SVG file are exactly the same file as the first uploaded file (eventhough the file name is different)

Problem/Motivation

When create multiple icon/terms, the SVG file are exactly the same file as the first uploaded file (eventhough the file name is different).

Steps to reproduce

- Add term on "icon" vocabulary
- Put name & symbol ID
- Upload svg file
- Save the newly created term
- Add another term on "icon" vocabulary
- Put name & symbol ID
- Upload svg file
- Right click on the file link and Open New Tab and you can see the svg is identical as the previous term's svg eventhough the file name is different)
- Save the term

On Node Edit/Creation Form:
- Create taxonomy reference field linked to "icon" vocabulary
- Select "icon select" widget on Manage Form Display
- All icons are the same (as the first uploaded svg)
