Add permission to bypass file caching

Problem/Motivation

While the PDF file caching feature is great in most cases, this is an obstacles editors who can control printable elements (e.g. hide/show paragraphs and/or HTML elements in print versions) would need to preview their editorial work-in-progress.

Proposed resolution

Add a bypass pdf caching permission and tweak the cache logic in BookPdfGenerator::getFileUri.
