Performance: Add db indexes

This is intended as part of the 8.x-2.17 release.

The columns should not be renamed. Renaming columns should not happen in minor releases, only a new 3.x branch

This issue was resolved. A new release, 8.x-2.17, will be made soon, May 31st.

This is an excellent opportunity to beta test the dev branch, and report any issues.
