Option to skip loading all forms

Problem/Motivation

For Hubspot user with many forms, the process to load all the forms will cause performance issue and break with timeout.
cURL error 28: Operation timed out after 5000 milliseconds with 23701759 bytes received (see https://curl.haxx.se/libcurl/c/libcurl-errors.html)

In order to make this module usable for this case, we should allow user to skip the loading and input the form GUID directly.

Proposed resolution

Add an option to "Skip loading the full list of Hubspot forms".

User interface changes

When skip loading is checked, the form widget automatically changed to textfield.
The format to use is "Portal_ID::GUID", e.g. 62515::233bb53d-071f-4d39-8859-f041855bbbd4'
