We are having this same issue on the latest 4.x.x release. Toggling "Distinct" in the views UI is not altering the query as expected. Any ideas where we would need to make a code adjustment to reduce duplicate rows?

One screenshot shows the regular Group Media View, where the correct number of images is displayed. I have inserted the selected image via Image Paragraph using a node.
When I am in the group media view, the new added image is output twice. I would therefore have to activate DISTINCT for the Group Media View so that there is no duplication.
I have not configured anything special, my setup looks like this:
* Group
** Subgroup
*** Node in the subgroup with the image paragraph
Cardinality for Group Media is set to 0, so it is not limited.
