Update form to use ajax submit so validation errors can be shown in the off canvas form.

When submitting the quick transition form if we receive any validation errors the user is redirected to an unstyled page at /moderation-sidebar/{entity_type}/{entity}. Update the form to use ajax submission so the user is shown any errors inside the off canvas form.
