How to add a set of text fields to a Quiz?

Hi

Thanks for having me in this community and thanks for your hardwork. Quiz is one of the most impressive and solid modules I've ever came across.

What brings me here is that I don't want to use Quiz as a learning platform, but more as a consultancy tool.

In my mind, every user will take as many quizes as they'd need (unlimited quiz settings->OK).

And every quiz will be a set of yes/no questions (true/false question ->OK)

And every quiz must start with a fixed set of input fields (type text fields) that users will have to fulfill in order to take a quiz... (MY ISSUE).

I've tried several things to make this happen already, but none of them have worked for me...

1- CONTENT TYPES: I've added a text field to quiz content type, but I can only see the input field in my admin page, not in the actual quiz.
2- QUIZ FIELD module.
3- WEBFORMS module

Please, can you help me find a solution here.

How can I add a set of input fields to a Quiz so that the user can do the quiz unlimited times but everytime he must enter different data?

Also, how can I show this data on the results page?
