I have created an amazon_hb module to add the required JavaScript for Amazon Header Bidding.
As per https://vimeo.com/259079860 and https://gist.github.com/cassy1003/271ba919e8e3e4a67e07af0d1390c698, there should be a googletag.pubads().disableInitialLoad(); line as JavaScript required for header bidding does its own refresh.
