Slow query in quiz_build_question_list

Hey hi,

We run a fairly big site which makes extensive use of the quiz module and had an issue with a slow query, The query in question was in the quiz_build_question_list function because a statement using an OR operator cannot make use of indexes.

I've split up the query to two parts using a UNION clause.

With D7 officially EOL going through and triaging the D7 queue of quiz. If still an issue with the latest 7.0.x branch (D10 + D11) please reopen
