Error when batching is off; minor improvements.

When batching is off, the batch_get function doesn't return anything and generating output is impossible.

Additional minor issues:

• Sometimes file is not saved due to write permissions issues and the end user still gets a success message.
• $variables['separator'] is not really used as the csv separator.
• HTML entities should be decoded when 'strip tags' setting is on to ensure clean output.
