Port hreflang tokens to D8

Port the dynamic node language tokens to D8; see #2597301: Expand hreflang support, auto-add fields for each locale → for how they work in D7.

Add the [node:url-original] token - provided in #6 but does not use the language of the original node.
Add separate [node:url-LANGCODE] tokens for each language - provided in #6, but needs to be expanded so it always adds the tokens, not just when there's more than one language installed.
Test coverage.
