Allow adding notes to each checkpoint

It would be a great (optional) feature to allow users to add description text / textual details to each checkbox.

An idea about how it could work:

• There should be a configuration option to enable descriptions for checkpoints.
• When it's enabled by configuration then a "description" link should appear next to each checkpoint.
• When a user clicks on the "description" link, a textarea should appear.
• The module sho

uld store the text entered to the description textareas.
