Fivestar layout appearance crashes on bootstrap theme

@agentrickard: Yes, if there is still a problem using this module with BigPipe please open a new issue with instructions for reproducing the problem.

Patch fails to apply because it's in the wrong format (not in -p1 format).

Here's a re-roll of #9 against the current HEAD. I am using #9 instead of #11 because #11 adds some bootstrap-specific CSS that I don't think should be committed as part of this module.
