Appreciate im a little late to the party, and not like for like as using 7.78 but ive tried to recreate this and struggling.
1. Drupal 7.78
2. Latest version of this module
3. Copy Adminer as per readme
4. Copy plugins as per readme
5. Copy styles as per readme
6. Check the config in admin/config/development/adminer/settings
7. Go to admin/config/development/adminer= Presented with a login form. Attached screenshot.
