Instead of just comparing the name of the file as seen in the Drupal UI, how about comparison of the resulting file system name as well?
In the case of identical uploaded filenames Drupal will append something like "_0", eg: testfile.txt and testfile_0.txt. In one node edit session I have removed the second testfile and uploaded a third that turned into testfile_1.txt, but Diff isn't indicating any change.
