user id and password for "user cache entries" ... where to set those parameters?

I am closing this support request because none of the mentioned files are part of this module. They are not even files required from this module to work.
If a user ID / password is required for those files to work, you need to read the documentation for MAMP or that apc.php.inc file. I can only guess apc.php.inc shows a page that should be seen only from authorized people, and that is the reason it asks for identification credentials.
