Would it make sense to include a select list of all the flags with the requisite prefix on the edit form for the message type? In that way the admin can select which flags to filter users on per message type.
A simple scenario, two flags are set on a question type. One is to subscribe when new answers arrive, another when comments are added. Currently, as I understand it, both flag user groups would receive notification of either event, unless one implements hook_message_subscribe_get_subscribers_alter.
