Looking at the ATAG standard B3 it would be nice to have a filter which suggests that users add headings.
This could be based on the length of the content contributed, use of bold or other elements which "look" like they could be headings.
Certainly if there are 5+ paragraphs it would be reasonable to assume that the content author add a heading.
This should be simply a recommendation popup. There is no way to automatically insert this.
I also think it should be off by default.
This might be something that needs to be added to a contrib module.

It would be a good tool for general readability too. Nobody likes a wall of text without headings.
