[tsm] manual review

• Delete master branch
• Document hook_galleryformatter_styles() in a gallerysformatter.api.php file.
• In galleryformatter_getimage_dimensions(), what's does the 4 mean? Can that be a constant instead?
• $item['filename'] = $item['filename'] = ''; looks like a copypasta error?

Didn't find a lot, clean code!
