Unpublished content and attachments should be kept private

Follow up for #1776796: Provide a better UX for creating, editing & managing draft revisions. →

Idea:

If content is published, move the attachments from private to public
If content is unpublished, move the attachments from public to private
Uploading attachments to all non-published revision states should be kept private

This allows us to keep top secret information really secret and do not allow prying eyes to non-ready content. This includes all marketing events that may require a fixed publishing date.

Not yet done: "check the nodes/entities using the media, not just the media entity" not implemented yet.

OK, I have a complete solution but in an early stage of development, the File Visibility → module.

Solution from #24 is abandoned.
