Required Fields showing up as NULL in database after ticket submitted.

We have four random tickets 2147, 2158, 2190, and 2224 all submitted on different dates (in the past month) that appear to not have captured the Required fields (name, email, tel#) in the database (all NULL). However, the non-required fields (Comments etc) from the ticket have been captured in the db.

We've had the ticketing system up and running for 2+ years and this is the first time we are experiencing this issue.
Has anyone else encountered this problem? We have tried to recreate it but with no luck. Other than the tickets were opened on a Monday or Wednesday, we dont see any other commonalities between the tickets.

Has anyone had this issue before and what was the root cause/solution.
