I know this is old but here's a patch that seems to work for me.
TOC is not working with panels panelizer, etc. I don't know which side the issue is with but for some reason the headers don't get any ids when I display the content in a panel or using Panelizer. It works fine with a regular node and using the TOC block. I've tried to display the full node with "extras" option unchecked in page manager but the id's don't show up still. I turn off panels or panels and its works again.
