Cleanup subgroups hierarchical select module

Here are some cleanup suggestions I have for hs_subgroups

* Renaming it to og_subgroups_hs
* Changing the package from "Form Elements" to "Organic Groups"
* Taking it out of the /modules folder. It can sit in the main directory until you have so many modules that a /modules directory is helpful.
