ad_hosts table doesn't exist

A few months ago, I did something stupid that nearly wrecked my site. I inserted Javacode into the field for external ad url. The result was disastrous and fixing it wasn't easy. The only way I could fix it was to erase the ad modules using FTP, which, I know, is not the proper way to uninstall the ad. But it was the only thing I could think of to unlock my website. And in that respect, it worked. Since then, I've tried to install and correctly uninstall ad module, but I continually run into errors.
