Modal pop-up to replace JS prompts

Problem/Motivation

The JS prompts to capture inputs for notes are not very user friendly.

Steps to reproduce

Try adding new notes or edit any note, the JS prompt would be triggered to add/update the values.

Proposed resolution

Leverage the modal pop-up to capture inputs from the users(reviewers).
