Fix performance issue in Migration module for users with many accounts

The performance of the “Migrate server” form becomes terrible for users with a lot of SearchStax accounts since the form code first loads the list of accounts and then, for each of them, the list of apps.

Instead, we should first let users pick an account and then only load the apps for that specific account.
