Improve support for users with multiple roles with amswap menus configured

Problem/Motivation

I have a user with 2 admin roles. Each role has a menu configured with amswap. The roles overlap in abilities, so their admin menus share many links.

The problem is that the user gets the entirety of both menus in their toolbar so they get many duplicate links. This makes for a confusing experience.

Steps to reproduce

1. Create 2 admin menus.
2. Create 2 roles
3. Create a user with both roles
4. Configure amswap so that the 2 roles each have a different admin menu
5. Log in with the user with both roles and see both menus in the toolbar
